#1 Filling out this form will book you and your estimated due date on my calendar to receive a placenta encapsulation service with Tara Carpenter, CPES. I will be on-call to encapsulate within 72 hours of your baby’s birth.
#2 Before filling out the form below, please see $50 non-refundable deposit and agreement.
#3 Once I receive this form, I will email a confirmation email within 3 business days, including a packet of informative handouts. I will also send an invoice via PayPal which can be paid with any major credit card (don’t need account) or personal check made payable to ‘Happy Bellies’.
- Full payment is preferably due 3 weeks prior to estimated due date.
- If you used cigarettes or recreation drugs during pregnancy, contact Tara before filling out this form.
- I often travel long distances to offer this service. There may be times I ask you to freeze your placenta briefly (1-3 days) if I am unable to encapsulate within 72 hrs. of birth.
